We would like to know how we can help you if you have a disability.

A passport with details about you, including what you like and do not like, will help us to care for you.  If the staff are able to use the passport you will not have to say the same things to everyone you meet in the hospital. 

Your carer or someone you trust can help you to fill in your passport before you come into hospital. When you do come to the hospital, please tell the staff you have a passport.
