Leaving hospital

When you come into hospital it is important that planning for your return home starts early in your stay and that it is well planned.

Your ward team will start to arrange your discharge from the day of your admission. This will ensure you have a smooth transfer from hospital to home. To help with this process we ask that you let your nurse know as early as possible of anything that may influence your ability to go home.

It is important that we work with you to organise your discharge plan early in your stay so that you do not remain in this acute hospital longer than is absolutely necessary. This is important for you so that you can return to your home as quickly as possible which has been shown to benefit your recovery. It is important for us so that we have beds available to be able to continue to provide acute hospital care for those who need it the most, as you did.
